The Community Navigator helps people access local community and state resources, benefits and assistance programs. The navigator provides  information and referrals to a wide array of professionals in particular specialties.

WHAT THE COMMUNITY NAVIGATOR CAN DO FOR YOU:

  • Listen to your needs

  • Refer you to local and/or state services that can help with your situation

  • Provide release of information forms for all contact with any person outside the program.

  • connect you to local case managers if applicable

  • Maintain confidential records of your information

WHAT THE COMMUNITY NAVIGATOR CAN NOT DO FOR YOU: 

  • Function as a caseworker 

  • Provide in-home support 

  • Perform counseling 

  • Distribute funding
